How to Register
Roundwood Park Medical Centre is proud to be a safe surgery for everyone in our community.
In recognition of the barriers to healthcare access faced by people in vulnerable circumstances, including migrants, we commit to protecting the human right to health, and will take steps to ensure that everyone in our community may fulfill their entitlement to quality healthcare.

If you would like to register with our practice, you’ll need to complete a GMS1 form and patient questionnaire.
We also ask for proof of ID and address so we can make sure we get your details right, and link any existing medical records you’ve got from a previous GP practice, but these are not mandatory and not being able to provide these won’t prevent you registering.
If you live outside of our catchment area, then we may still be able to register you as an ‘out of area’ patient. This means we wouldn’t be able to provide home visits if you required them and you should be sure this is suitable for your individual circumstances before registering. 2. Complete the patient questionnaire below and email a copy to us at breccg.e84656registrations@nhs.net . Sometimes the questionnaire downloads can be blocked by your internet browser – copy and paste the full link into your address bar if this is the case.

Now email a copy of your completed questionnaire along with your ID and proof of address to breccg.e84656registrations@nhs.net (or come into the practice to provide these in person)
Out of Practice Boundary Area Registration
Where it is clinically appropriate and practical to register, we now accept new registrations from patients outside of our usual practice catchment area.
Patients registered this way would not be entitled to a home visit from the practice. However, if you live outside the practice area and need a home visit, please contact NHS 111 in order to be seen by a practice closer to where you live.
If you have become permanently housebound and live outside the practice catchment area area, the practice advises that you register with one of our GP colleagues closer to where you live. We believe this is in the patients best interests.
Registering for Online Services
You can use the Internet to book and cancel an appointment with a GP, request repeat prescriptions and view aspects of your medical record online. You’ll need to provide proof of ID, or come in to the practice to do this in person if you don’t have ID.
